Elyse Giuffre is the SCI Navigator program administrator in the Center for Spinal Cord Research at Kessler Foundation. She provides patient-centered education and supportive resources to spinal cord injury patients and their families as they transition from acute care to their discharge location and from inpatient rehabilitation to home. Elyse is certified in New Jersey as a clinical social worker, a social worker supervisor, and an Enhanced At-Risk Criteria (EARC) Pre-Admission Screening (PAS) Assessor. She previously worked as director of case management focusing closely on coordination of care, throughput, and end-of-life procedures at Kindred Hospital. She also worked as a staff social worker at Saint Barnabas Medical Center focusing on patient assessments, patient/family education, interdisciplinary management, and discharge planning.
